I might go with Willow from Buffy. But I do admit to a certain, erm, bias.
Favorite Witch? Glinda, the Good Witch of the South. Book version, naturally.
I have to go with Helena Markos from Suspiria. She wasn't in the movie physically that much, but her presence was felt throughout the film.
My favorite witch would have to be the Sanderson Sisters from Hocus Pocus.
Howl from Howl's Moving Castle is my current favorite, but Avatar from Wizards close.

Favorite wizard is One-Eye from the Black Company.
Right at this moment, my favorite witch is Jessica Lange in the current American Horror Story. I will sit and watch her be bitchy all day long, and I don't know why. In general, if my wife's watching a cute-girls-being-witches show, I'll be compelled to sit and watch. It's a weakness.

My favorite Wizard/Warlock is Julian Sands' character from the movie Warlock (1989 - he played the Warlock).
My favorite Witch is Princess Asa from Black Sunday (1960).
I'm going with Baba Yaga right now, but Morgan le Fey is decent. Fictionally, probably Sun Wolf (The Ladies of Mandrigyn & related books, by Barbara Hambly.) I might have others but I'd have to do some research. Menw fab Teirgwaedd, from Culhwch & Olwen, is cool - not described much, but I like the whole adventuring party aspect of the story.
